If you are a new patient scheduled for a FREE LASIK evaluation, please print and complete the following forms and bring them to your appointment:

• Patient Demographics
• Designated Party Release

For all other new patients (except LASIK patients), please print and complete the following forms and bring them to your appointment:

If you have Medicare, please complete the following form:

• Medicare Patients

If you do not complete these forms in advance, please arrive at least 30 minutes prior to your appointment to avoid unnecessary delays.
